Church Hill Medical Mission is a clinic that serves the uninsured. Our mission statement is:

Our mission is to demonstrate the love of Jesus Christ by providing free, quality medical care to individuals in need while promoting holistic health—physical, emotional, and spiritual.
Providing general medical care and Christian support to uninsured residents of Hawkins and Hancock Counties and the Tri-cities community in Northeast Tennessee; healing physically and spiritually as Jesus did.

Eligibility Guidelines:
We serve patients who are uninsured and whose incomes fall within 200% of the federal poverty level.

For a household of 1: $29,160

For a household of 2: $39,440

For a household of 3: $49,720

For a household of 4: $60,000
Applications can be picked up at the clinic on Monday through Thursday from 10 a.m. – 3:00 p.m. You may also download a copy below or request one be sent to you.
Applications must be returned in person along with a valid photo ID and proof of income.

Clinic Schedule: The clinic is open on the second, third and fourth Tuesday each month for morning & evening clinics. Morning Clinic is from 9-3:00 and evenings from 5 – 8:30 p.m.

Beginning January 2026 we are adding clinic hours on Wednesday morning from 9-12:00
