Church Hill Medical Mission is a clinic that serves the uninsured. Our mission statement is:
Providing general medical care and Christian support to uninsured residents of Hawkins and Hancock Counties and the Tri-cities community in Northeast Tennessee; healing physically and spiritually as Jesus did.

Clinic Schedule: The clinic is open on the second, third and fourth Tuesday evenings of each month from 5 – 8:30 p.m. Doctors start seeing patients at 6 p.m. Patients are seen by appointment only, limited to the first 25 patients. Call us at 423-256-2408 to make your appointment.

Eligibility Guidelines:
We serve patients who are uninsured and whose incomes fall within 200% of the federal poverty level. Please click here to determine if your income is within these guidelines.
You must apply to become a patient at the clinic.
Applications can be picked up at the clinic on Monday through Thursday from 10 a.m. – 3:00 p.m. You may also download a copy or request one be sent to you.
Applications must be returned in person along with a valid photo ID and proof of income.
